The presentation will review a standardized approach to “handoff” communication, including an opportunity to ask and respond to questions, in order to ensure communication of the patient’s status and care between providers and others. The objective of the “handoff” is to provide accurate information about a patient’s care, treatment and services, current condition and any recent or anticipated changes.

The hand-off/transfer process will provide for the safe and timely transfer of the patient. Up to date information regarding the patient’s care, treatment, medications, services and any recent or anticipated changes will be included.
